The 697 came from 4 years ago when the guy was probably still a learning junior - he travelled out of the US for a few years and came back recently and played this event. Other examples are less stark because the initial rating is rarely so low, but they exist - there was a guy from Trinidad (I think) who recently played at WestChester and won virtually every even from the U2050 up to the U2400 because he was rated 2021 or so when he last played in the US which was a couple years back.
Larrytt had an extensive blog post on him as he plays/played at MDTTC.
